29 CFR 1926.451(f)(4):     Any part of a scaffold damaged or weakened such that its strength is less than that required by paragraph (a) of this section was not immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired.      a)  Mobile scaffolding/scissor lift, Model Skyjack CBS 279661, Interior of Job site:  On or about 04/11/2012, and at times prior thereto,  An employee was exposed to a fall of approximately 12' 6" to the concrete ground below.  The employee was operating a scissor lift without having the midrail access chain attached.  The chain/hook was broken and not capable of being attached.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level.     a)  Mobile scaffolding/scissor lift, Model JLG 1930ES, Interior of Job site:  On or about 04/11/2012, and at times prior thereto,  An employee was exposed to a fall of approximately 12' 6" to the concrete ground below.
