29 CFR 1926.451(b)(2)(i):     Each ladder jack scaffold, top plate bracket scaffold, roof bracket scaffold, and pump jack scaffold platform was not at least 12 inches (30 cm) wide:     a) On or about 04/16/12 CSHO observed two men working on and from Alum-A-Pole pump jack scaffold when one of the walking/working platforms (2 x 8 timber) being used was not at least 12 inches wide.
