29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. (a) Jobsite: 437 Loyd Avenue, Latrobe, Pa 15650; On or before April 12, employees were working unprotected and exposed to fall hazards of approximately 18 feet 6 inches to ground level while on a 5/12 pitched roof. The employees removed an old roof and were replacing the sheeting.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite: 437 Loyd Avenue, Latrobe, Pa 15650; On or before April 12, employees did not receive training from their employer on the hazards associated with falling while removing and applying roofing materials on a steep sloped roof and what fall protection methods shall be used. Employees were provided with small diameter rope with a breaking strength of approximately 300 Lbs. to be used as their connection device without a deceleration device.
