OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY:  The load and/or lift components should the lift fail.    a)  At the facility, on or about 04/05/12:  Fourteen surface mounted automotive lifts were being operated without their swing support arm locks being operational.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to:    Ensure automotive lifts are operated in accordance with the Automotive Lift Institute, Inc. standard for automotive lifts, aaragraph 4.4.2 which states; The operator shall use all applicable safety features provided on the automotive lift, and operate the lift in accordance withthe instructions furnished by the lift manufacturer.    b)  At the facility, on or about 04/05/12:  Three in-ground automotive lifts were leaking hydraulic fluid.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to:    Ensure hydraulic lifts are inspected and/or repaired in accordance with the Automotive Lift Institute, Inc. standard for automotive lifts, paragraph 5.6.3.1 which states; Report any hydraulic leaks or air leaks.  Additionally, emsure operators follow section 4.5 which states; If any of the conditions listed in 4.4.4 are observed, specifically in this instance, the evidence of hydraulic leaks, then the lift shall not be used until the cause of the problem has been determined and repairs have been made...    c)  At the facility, on or about 04/05/12:  The automotive lift in maintenance bay 12 does not lower evenly.    Among other methods, one feasible and acceptable method to correct this hazard is to:    Ensure all lifts operate properly in accordance with the Automotive Lift Institute, Inc. standard for automotive lifts, paragraph 4.4.4(4) which addresses proper operation of the lift controls, synchronizing systems.  Additionally, operators must follow section 4.5 which states; if any of the conditions listed in 4.4.4 are observed, then the lift shall not be used until the cause of the problem has been determined and repairs have been made...    d)  At the facility, on or about 04/05/12:  The tire changing machine had an inoperable table top and clamp control  peddle.    Among other methods, one feasible and acceptable method to correct this hazard is to:    Ensure the tire changing machine is maintained and operated in accordance with the manufacturer's operating instruction manual.    Abatement certification must be submitted for these items.
