29 CFR 1926.453(b)(2)(v):  A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.        On or about April 16, 2012, on the north side of the building, an employee engaged in painting activities on a commercial building from a man-lift without fall protection was exposed to the hazard of falling approximately 15 feet to the ground.

29 CFR 1926.503(a)(2)(i):  The employer did not ensure that each employee was trained, as necessary, by a competent person qualified to recognize the nature of fall hazards in the work area.        On or about April 16, 2012, an employee engaged in painting activities on a commercial building from a man-lift without a body belt and lanyard was exposed to the hazard of falling approximately 15 feet to the ground.  The employer did not provide fall hazard training to the employee.
