29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems.        On or about April 16, 2012, employees engaged in roofing activities on a residential building with an 8:12 pitch, worked without fall protection and were exposed to the hazard of falling approximately 8 feet to the lower level.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.      On or about April 16, 2012, on the east side of the building, employees engaged in roofing activities on a residential building utilized an extension ladder that did not extend 3 feet above the landing were exposed to the hazard of falling approximately 8 feet to the ground below.
