29 CFR 1926.761(b): Fall hazard training. The employer shall train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program. The employer did not initiate a training program designed to ensure that employees engaged in steel erection operations were trained to recognize fall hazards and acquire the knowledge to abate them. Employees were exposed to fall hazards up to approximately nineteen feet to ground.

Employees were observed performing steel erection operations from a scissors lift without safety chains being used Employees were exposed to a fall hazard of approximately twelve (12) feet to the concrete surface below. 2. Employees were observed performing steel erection operations from a scissors lift while standing on the mid-rail and top-rail of the lift. Employee were exposed to to a fall hazard of approximately fourteen (14) feet to the concrete surface below.
