29 CFR 1926.761(b): Fall hazard training. The employer shall train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program.    The employer did not initiate a training program designed to ensure that employees engaged in steel erection operations were trained to recognize fall hazards and acquire the knowledge to abate them. Employees were exposed to fall hazards up to approximately nineteen feet to ground.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    At the jobsite, employees were observed not wearing hardhats while work was being conducted overhead.  Employees were exposed to struck by hazards.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    1. Employees were observed performing steel erection operations from a scissors lift without safety chains being used  Employees were exposed to a fall hazard of approximately twelve (12) feet to the concrete surface below.      2.  Employees were observed performing steel erection operations from a scissors lift while standing on the mid-rail and top-rail of the lift. Employee were exposed to to a fall hazard of approximately fourteen (14) feet to the concrete surface below.

The employer did not ensure that employees performing steel erection operations were provided and utilized fall protection.  Employees were exposed to a fall hazard of up to nineteen (19) feet to the concrete surface below.
