29 CFR 1910.67(c)(2)(v):   A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  On or about February 23, 2012, a worker was removing a sign while elevated up to approximately 20 feet high in a Genie Z-30 aerial lift without utilizing fall protection, exposing the employee standing on the ground to the hazard of being struck by a falling worker.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that workers elevated above employees are required to use fall protection.

29 CFR 1910.135(a)(1):   The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  On or about February 23, 2012, an employee was standing below a worker in an aerial lift without head protection while the worker was removing a sign with the use of a drill, hammer, putty knife and sockets, exposing the employee to the hazard of being struck by falling objects.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee exposed to overhead hazards are provided and utilizing head protection.
