Lawn & Garden - The employer failed to obtain the manufacturer's written approval when using a Yale fork truck (SN B875B08405D - Lowes ID  F28154) with added fork extensions.     Lowes Home Centers, Inc was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 314245267, citation number 1, item number 3a, issued on 10/03/2011, with respect to a workplace located at 850 East High Street, Carlisle, PA 17013. The final order date for the citation was on 2/21/2012 upon formal settlement agreement.    Lowes Home Centers, Inc was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 313795346, citation number 1, item number 1, issued on 2/01/2010, with respect to a workplace located at 850 Route 44, Raynham, MA 02767. 29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a.  Lawn & Garden - Fork extensions are used with a Yale fork truck (SN B875B08405D - Lowes ID  F28154) to move fencing and the forklift did not have capacity plates reflecting the use of the extensions.
