29 CFR 1926.453(b)(2)(iii):     Employees were not prohibited from belting off to an adjacent pole, structure, or equipment while working from an aerial lift:    (a) Mechanical Room, Penthouse, High-Bay Areas of the Building - An employee, working out of a JLG aerial lift, welded steel for a skylight three stories above the ground.  The employee tied off to a beam of the building rather than to the aerial lift, on or about April 2, 2012.

29 CFR 1926.453(b)(2)(iv):     Employees working in aerial lifts were not standing firmly on the floor of the basket:  (a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)      (a) Mechanical Room, Penthouse, High-Bay Areas of the Building - Three employees, each working out of separate JLG aerial lifts, set and welded steel three stories above the ground.  These employees stood on the midrails and top rails of the aerial lifts rather than firmly on the floors of the baskets of the lifts, on or about April 2, 2012.

29 CFR 1926.1053(b)(4):     Ladder(s) were used for purposes other than the purpose for which they were designed:  (a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)    (a) East Side of Building - The crane operator used a 6-foot step ladder leaned in a closed position to access the crane, on or about April 2, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
