The employer did not record the following workplace injuries on an OSHA 300 Form (or equivalent form) for Calendar Year 2012:    a)  On February 3, 2012, an employee's finger was injured on a power washer, resulting in days away from work.  b)  On March 5, 2012, an employee incurred a hernia while lifting, resulting in days away from work.  c)  On April 4, 2012, an employee's finger was lacerated by a knife while fileting fish, resulting in medical treatment.      NO ABATEMENT CERTIFICATION OR DOCUMENTATION IS REQUIRED FOR THIS ITEM.

The employer did not record the following two workplace injuries on an OSHA 300 Form (or equivalent form) for Calendar Year 2011:    a)  On 2/2/11, an employee's finger was lacerated on a hand tool, resulting in days away from work.  b)  On 3/31/11, an employee slipped and injured his back, resulting in days away from work.  c)  On 4/13/11, an employee incurred a puncture wound to the thigh, resulting in days away from work.  d)  On 5/8/11, an employee (truck driver) was injured in an vehicle accident, resulting in days away from work.  e)  On 7/8/11, an employee's (truck driver) foot was fractured, resulting in days away from work.  f)   On 8/29/11, an employee incurred a chest injury after falling on a hand truck, resulting in days away from work.  g)  On 9/20/11, an employee slipped on a customer's dock and broke his wrist, resulting in days away from work  h)  On 12/23/11, an employee slipped on the floor and injured his back, resulting in days away from work.    NO ABATEMENT CERTIFICATION OR DOCUMENTATION IS REQUIRED FOR THIS ITEM.

29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by this paragraph (l).  The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.      The employer did not certify that a performance evaluation (driving test) had been conducted at least once every three years for all authorized powered industrial truck operators.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
