29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.      a)  At the residential construction site: On April 25, 2012; the employer failed to ensure that employees were protected from eye injuries by issuing and ensuring that employees wore safety glasses. The employees were expposed to eye hazards from splintering wood or nail fragments while operating a pneumatic framing gun to install corners.

29 CFR 1926.451(g)(4)(i):     Guardrail systems were not installed along all open sides and ends of platforms.      a)  On the north side of the residential construction site:  On April 25, 2012; the employer failed to install a guard rail system along the 8 foot opening at the front of the man basket, exposing employees to falls of at least 17 feet while installing corners from the elevated man basket.
