29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:      a)  Employee was not provided safe access/egress of the working level of the scaffolding system.    **  No abatement certification or documentation is required for this item.**

29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a)  Employee was not protected from falling from the working level of the ladder jack scaffold.  Employees were exposed to falling at 13ft to the ground below.    **No abatement certification or documentation is required for this item.**
