29 CFR 1926.20(b)(2):     The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).    At the jobsite located at 721 Kirkwood, Springfield, IL, employees performing residential roofing activities were exposed to fall hazards and the employer failed to ensure that frequent and regular inspections of the jobsite were made to by a competent person to identify and eliminate the hazards.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    At the jobsite located at 721 Kirkwood, Springfield, IL, an employee was exposed to a fall hazard of approximately 30 feet  to the ground below while performing residential roofing operations on a roof with a pitch of approximately 8/12.  The employee was not provided with fall protection.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    At the jobsite located at 721 Kirkwood, Springfield, IL, an employee was exposed to a fall hazard of approximately 30 feet to the ground below while performing residential roofing operations on a roof with a pitch of approximately 8/12.  The employer had not provided a training program to train employees on fall hazards and fall protection.
