5(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer furnish employment and place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: On or about April 18, 2012, and at time prior thereto, in the small and big bays load hooks of overhead cranes were observed not having safety latch provided. This exposed the employee to the dropped on and/or struck by hazard.

29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not clearly marked for identification of functions: On or about April 18, 2012, and at times prior thereto, in the small and big bay areas overhead crane pendant control boxes were observed having some control buttons not clearly marked for identification and function. The employees were exposed to hazards of dropped on and/or struck by materials.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 18, 2012, and at times prior thereto, in the roll machine areas the employer allowed employees to operate a roll machine which had emergency trip device provided only to one side of the roller. Employees were exposed to amputation hazards.
