29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each hole.    (a) On or about 04/18/2012 at 221 Catherine Street, Union, NJ 07083: Employees were exposed to fall hazards of approximately 9'2" when holes in the stairway landing were not covered. Employee fell through finished sheetrock ceiling below on the access stairs located 9'2" below.
