29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Econolodge addition Roof - On or about 4 May, 2012 - During roof shingle installation, employees were installing asphalt shingles on a 5 on 12 pitched roof deck without utlizing conventional fall protection. Employees were exposed to a fall hazard of 32 feet.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    a) Econolodge addition stairwell - On or about 4 May, 2012 - Employees were using a stairwell that was not equipped with a handrail. Employees were exposed to a fall hazard of 10 feet.

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:    a) Econolodge addition third floor landing - On or about 4 May, 2012 - Employees using the stairwell were exposed to a fall hazard at the third floor landing because the guardrail was not capable of withstanding a force of at least 200 pounds applied at the top edge. Employees were exposed to a fall hazard of 10 feet.
