29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    (a)  Roof  - Two chimney mechanics were exposed to a thirty (30) ft. fall from the unprotected edge of a low-sloped roof while installing a new flue liner in a chimney without fall protection, on or about 04/20/2012.

29 CFR 1926.1053(b)(1):     Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support:  (a)  Roof - Two chimney mechanics were exposed to an eight (8) ft. fall from the top of a portable aluminum ladder which did not extend three (3) feet above the ladder landing and was not secured in place and provided with a grabrail, on or about 04/20/2012.
