29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Workers were exposed to fall hazards from open sided floors from the second floor, third floor and fourth floor on March 27 and 28, 2012. (b) Workers were exposed to fall hazards from open sided floors from the second floor and third floor on March 14, 2012. (c) A worker fell from the third flloor from where the side of the floor was left open on March 13, 2012.
