29 CFR 1926.102(a)(1): Employees were not provided with eye and face protective equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:                                                                                   a)  On or about May 1, 2012 and at times prior, employees using hand tools and/or pneumatic nail guns while re-roofing a roof were not wearing adequate eye protection that meet the requirements of ANSI Z87.1  Employees were not wearing safety glasses.    Abatement Note: Abatement certification is required for this item. (see enclosed "Certification of Corrective Action" Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:    a) On or about May 1, 2011, employees were exposed to fall hazards in excess of 9 feet, while installing the new roofing without fall protection. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:    a)  On or about May 1, 2012 and at times prior, employees using an portable ladder to access the roof area were exposed to fall injury in that the ladder was not secured in place nor did the side rails of the ladder extend at least 3 feet above the upper landing surface.
