29 CFR 1910.179(n)(1):     Crane(s) were loaded beyond the rated load (maximum load for which the crane or individual hoist was designed and built by the manufacturer and shown on the equipment nameplate(s))    On or about 04/25/12 an employee was exposed to a struck by hazard while using an overhead crane to move heavy granite blocks weighing 14 tons, the crane load rating capacity was for 12 tons. The employee was exposed due to a possible failure whereby sticking the employee.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.184(c)(1):     Sling(s) that were damaged or defective were used:    On or about 04/25/12 employees were exposed to a struck-by hazard while working in the saw and polishing mill using a defective sling (torn) to lift slabs weighing 2,000 pounds. The employees faced possible rupture of the slings.    No abatement certification or documentation is required for this item.

29 CFR 1910.303(b)(7)(i):     Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment    On or about 04/25/12 an employee was exposed to an electrical hazard while operating the light switch inside of an electrical panel with an unused opening. The employee was exposed to an electrical shock. 29 CFR 1910.305(b)(2)(i):     Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    On or about 04/25/12 employees were exposed to live exposed wires hanging out of the junction box while operating the polishing mill. The wires serve to provide current to the hydraulic system and the travel mechanism for the mill. The employees are exposed to burns and electrical shock.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
