29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part. The employer did not develop a safety program for his employees that were exposed to falls during commerical roofing operations. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19 (c).

29 CFR 1926.501(b)(11): "Steep roofs" Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Employees working on a 10:12 pitch steep roof were not utilizing adequate fall protection, thereby exposing the employees to falls of 11.5 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19 (d).

29 CFR 1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head. The snap hook for a belt restraint system was not located in the middle of the back of workers while performing commercial roofing operations. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19 (c).
