29 CFR 1910.332(b)(1)  Employees were not trained in and familiar with the safety-related work practices required by 1910.33 through 1910.335 that pertain to their respective job assignments:    (a)	Master Chemical:  On or about May 3, 2012, the employer did not ensure that maintenance personnel performing work on an energized or potentially energized 440 volts AC 3-phase circuit providing power to the pump in the C tank were familiar with or trained on how to protect themselves from electric shock, arc-flash, and arc-blast hazards from energized electrical parts and equipment.  Personnel were not aware of safe approach distances, were not aware of the need for electrical personal protective equipment (PPE), and were not knowledgeable in the selection of proper PPE to protect from electric shock, arc flash, and arc explosion.

29 CFR 1910.333(a):  Safety related work practices were not employed to prevent electrical shock or other injuries resulting from either direct or indirect contacts, when work was performed near or on equipment or circuits which were or could be energized:      (a)	Master Chemical:  On or about May 3, 2012, and on a continuing basis, the employer did not ensure that maintenance personnel working on 480 volts, 3-phase circuits followed safe work practices.  Maintenance personnel were not aware of safe approach distances, did not utilize properly rated insulated tools, and were not aware of the need for electrical personal protective equipment.   Employee used a Fluke model 21 multimeter to troubleshoot a pump on C tank.
