29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Designers Mirror and Glass, Inc. on or about May 10, 2012, at the worksite located at 2500 Michigan St. in Sidney, OH: Employees performing window finishing operations from a flat roof were exposed to an approximate 11 ft. fall hazard.  The employer did not assure that unprotected sides were guarded to prevent falls.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
