New Braunfels Ave., San Antonio, TX, On or about 05/07/2012, and at times prior thereto, an employee was exposed to an electrical hazard while using power tools plugged into a defective extension cord, which had a missing a grounding pin that interrupted the path to ground.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  7803 S. New Braunfels Ave., San Antonio, TX, On or about 05/07/2012, and at times prior thereto, an employee was exposed to the hazards of falls from an elevation of approximately 113  to the dirt ground below, while cutting and removing flashing from a balcony without a form of a fall protection system.     THE HULL ASSOCIATES, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312117906, CITATION NUMBER 01, ITEM NUMBER 002 AND WAS AFFIRMED AS A FINAL ORDER ON 01/29/2009 WITH A FINAL ABATEMENT DATE OF 04/22/2009, WITH RESPECT TO A WORKPLACE LOCATED AT 3920 MCKINNEY AVE, DALLAS, TX  75204.
