29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions:      a) On or about May 3, 2012, at 3311 Navajo Peace, San Antonio TX, an employee was cutting plywood with a circular saw and was not wearing any type of eye protection.     b) On or about May 3, 2012, at 3311 Navajo Peace, San Antonio TX, an employee was working around a pneumatic nail gun and was not wearing any type of eye protection.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:    a) On or about May 3, 2012, 3311, Navajo Peace, San Antonio, TX, employee (s) were exposed to the hazard of falling off a portable 8 ft fiberglass A-frame self  supporting step ladder. The self  supporting  A frame step ladder was placed incorrectly leaning against the wall. The step ladder was improperly used and set up without following the manufacturers safety guidelines.
