On or 05/09/2012, inspection of the work site located at 504 Frair Court, Sherwood, Missouri was not conducted by a competent person. On this date, one of the two employees who were sent to deliver roofing materials was electrocuted when the conveyer boom made contact with an over-head power line.

The condtions were observed on the following trucks: 1) Truck Number 42. 2) Truck Number 43. 3) Truck Number 44. 4) Truck Number 45. 5) Truck Number 46. 6) Truck Number 47. 7) Truck Number 48. 8) Truck Number 57.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On 05/09/2012, employees delivering roffing supplies to a residential building located at 540 Frairs Lanre, Sherwood Missouri were not protected from electrical hazards. One of the two employees was electricuted when the truck mounted conveyor boom made contact with a 14.4.kilo volt overhead powerline. The employer dd not ensure that the overhead power line was deenergized or covered to protect the employees.
