29 CFR 1926.20(b)(2):     The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.    On or 05/09/2012, inspection of the work site located at 504 Frair Court, Sherwood, Missouri was not conducted by a competent person. On this date, one of the two employees who were sent to deliver roofing materials was electrocuted when the conveyer boom made contact with an over-head power line.

29 CFR 1926.21(b)(2):     The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    Each employee engaged in roofing supply delivery was not instructed in recognition and avoidance of unsafe conditions and regulations applicable to the duties they were assigned to. On 05/09/2012, an employee who was engaged tin roofing supply duty was electrocuted when the truck mounted over head power line.

29 CFR 1926.300(b)(2):     Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:  (a)(LOCATION)(DESCRIBE PARTS)(BELTS, GEARS, SHAFTS, PULLEYS, SPROCKETS, SPINDLES, DRUMS, FLYWHEELS, CHAINS AND ETC.)(IDENTIFY EQUIPMENT)    Operators, laborers and or drivers who were engaged in delivery of roofing supplies on to residential and commercial roofs, using truck mounted conveyers were not protected from injuries to their hands and fingers as the result of non-guarded chains and sprockets on the recieving (unloading end). The condtions were observed on the following trucks:    1) Truck Number 42.    2) Truck Number 43.    3) Truck Number 44.    4) Truck Number 45.    5) Truck Number 46.    6) Truck Number 47.    7) Truck Number 48.    8) Truck Number 57.

29 CFR 1926.416(a)(1):     Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    On 05/09/2012, employees delivering roffing supplies to a residential building located at 540 Frairs Lanre, Sherwood Missouri were not protected from electrical hazards. One of the two employees was electricuted when the truck mounted conveyor boom made contact with a 14.4.kilo volt overhead powerline.  The employer dd  not ensure that the overhead power line was deenergized or covered to protect the employees.
