29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    Employees are exposed to falls of 11 feet 8 inches to a lower level while standing on an open-sided platform used to access a fixed ladder on the corn silo.

29 CFR 1910.27(d)(2):     "Landing platforms." When ladders are used to ascend to heights exceeding 20 feet (except on chimneys), landing platforms shall be provided for each 30 feet of height or fraction thereof, except that, where no cage, well, or ladder safety device is provided, landing platforms shall be provided for each 20 feet of height or fraction thereof. Each ladder section shall be offset from adjacent sections. Where installation conditions (even for a short, unbroken length) require that adjacent sections be offset, landing platforms shall be provided at each offset.    Employees are exposed to falls while working on an approximately 80 foot high fixed ladder which did not have an landing platform at every 30 feet of height.

29 CFR 1910.307(c)(2):     Electrical installations were not approved for the class location, but also for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was present:     At the belt loading spout for Bin #8, where a Class II combustible dust was present, general service electrical equipment, including but not limited to Wayne Submersible Sump Pump, and a 110 volt receptacle were facilitated in lieu of Class II Division 1 ignition proof installations.   The hazard exposed employees to potential combustible dust deflagration.
