29 CFR 1926.21(b)(2):     The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    At the facility, on or about 05/04/12:  A temporary employee assigned to Destiny USA was injured while helping to unload a shipping container.  A stack of pallatized floor tile fell and struck the employee while inside the shipping container.    Abatement certification must be submitted for this item.
