Davis Highway Pensacola, FL 32505: On or about May 17, 2012 and at times prior to; the employer exposed his employees to falling or flying object hazard in that employees were allowed under an excavator with a suspended load while installing 14ft 8 inch diameter sewer pipes in a 9.5 ft trench excavation without a hardhat.

Davis Highway Pensacola, FL 32505: On or about May 17, 2012 and at times prior to; the employer exposed his employees to an engulfing/crushing hazard in that he allowed employees to access a trench excavation which measured 9.5 feet in depth and 25 feet in length without providing other safe means of egress.

Davis Highway Pensacola, FL 32505: On or about May 17, 2012 and at times prior to; the employer exposed his employees to a struck-by hazard in that he did not ensure employees who were installing 14 ft. 8 inch diameter sewer pipes in a 9.5 feet deep and 25 feet length trench excavation were not wearing warning vests while working near vehicular traffic.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the excavation was sloped at an angle steeper that one and one half horizontial to one vertical (34 degrees measured from the horizontal):    (a) 7171 N. Davis Highway Pensacola, FL 32504: On or about May 17, 2012 and at times prior to; the employer exposed his employees to crushing/engulfing hazards in that employees were allowed to install 14 ft. 8 inch sewer pipes in a trench that was 9.5 feet in depth and 25 feet in length (class B soil) which had nearly vertical excavation walls that were not sloped back to the proper angle as required by the elements of Sloping/Benching options (2), (3), or (4).
