On or about May, 11, 2011 at 246-12 Van Zandt Street, Little Neck, NY 11362    While installing shingles on a residential rooftop an employee was observed using a nailing gun without eye protection.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder���s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    A.) On or about May 11, 2012 at 246-12 Van Zandt Street Little Neck, NY 11362    Employees were observed accessing a residential rooftop using a portable ladder not extended at least three (3) feet above the upper landing surface exposing employees to fall hazards.
