29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) On the Phase I mezzanine level on or about May 22, 2012, and at times prior thereto, employees were not protected from falling about 15 feet to a lower level while they used hoses to spray water on demolished concrete rubble and dust.  Employees, near unprotected edges, stood and walked on chunks of concrete, pieces of bent rebar and twisted screen on an irregular shaped 25 foot by 25 foot portion of the mezzanine being demolished.  A large portion of a mezzanine, formerly used for a restaurant, was being demolished for a new casino entrance.   The concrete had been demolished with jack hammers mounted to skid steers.
