29 CFR 1926.451(e)(1) When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:  1)  At the jobsite, safe access to the ladder jack scaffold used by employee(s) to apply siding to the side of a residence was not provided.  Three supporting brackets were facing away from the gable end of the home.  Employee(s) were required to climb over these brackets to gain access and egress from the pick board.

29 CFR 1926.451(g)(1)(i)  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   1)  At the jobsite, a personal fall arrest system was not in use by the employee(s) who were applying siding to the side of the residence from a ladder jack scaffold.  The fall distance from the pick board toward the front end of the home measured approximately 17.3 feet.  The fall distance from the pick board at the back end of the home measured approximately 22 feet.
