29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above a lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:      a) Jobiste at 1281 Monestary Drive, Latrobe, PA:  On or about May 23, 2012, an employee was installing vinyl siding from a pump jack scaffold and the midrail was missing and there was not any fall protection onsite.  The fall hazard to the employee was approximately 14 feet to the ground.

29 CFR 1926.451(b)(5)(ii):  Each platform greater than 10 feet in length was extended over its support more than 18 inches and was not designed nor installed so that the cantilevered portion of the platform was able to support employees without tipping, nor had guardrails which block employee access to the canitlievered end:    a) Jobsite at 1281 Monestary Drive, Latrobe, PA:  On or about May 23, 2012, an employee was installing vinyl siding from a pump jack scaffold and the aluminum pic that he was working from extended over the pump jack supports by approximately 5.5 to 6.5 feet on both sides.  The fall hazard to the employee was up to approximately 14 feet.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection which meets the requirements of paragraph (k) of section 1926.502:      a) Jobsite at 1281 Monestary Drive, Latrobe, PA:  On or about May 23, 2012, an employee was exposed to an approximate 9 foot fall hazard while working from the garage roof.  The employee was installing siding on the house.

29 CFR 1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:    a) Jobsite at 1281 Monestary Drive, Latrobe, PA:  On or about May 23, 2012 an employee was using a 16 foot aluminum extension ladder to access the roof and this ladder only extended about 10 inches above the roof.  The fall hazard to the employee was approximately 9 feet to the driveway.
