1926.502(d)(15)    Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.    Jobsite at 1461 S. 12th, Omaha, NE, 68105-Worker was observed stripping shingles off a roof while using a personal fall arrest system that was not anchored to the roof in accordance with the manufacturers design.  The anchorage was installed incorrectly in that it was not secured to both sides of the roof ridge and was not completely attached to the roof deck.  In addition, the employer did not provide evidence that the anchorage, as installed, was capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.    Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
