29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a)	On or about May 22, 2012; in the Roto-Molder area of the jobsite, employees were allowed to utilize a port-a-cool (S/N-91927-05) fan which had an electrical plug that was not provided adequate strain relief.     b)	On or about May 22, 2012; in the Roto-Molder area of the jobsite, employees were allowed to utilize a portable cooler (S/N-C92) fan which had an electrical plug that was not provided adequate strain relief.

29 CFR 1910.334(a)(3)(i):     A flexible cord used with grounding type equipment shall contain an equipment grounding conductor.    a)	On or about May 22, 2012; in the Roto-Molder area of the jobsite, employees were allowed to utilize a port-a-cool (S/N-91927-05) fan which had an electrical plug with a ground pin that was plugged into a flexible extension cord which did not contain a grounding conductor.
