North side of building - employees were exposed to a 17'-4" fall hazard as the top and mid rails were not installed on the east and west side of the scaffold. Violation observed on or about 05/11/12.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a. North side of building - employees were exposed to an approximate 6'-6" fall hazard, in that, the top of the small A-frame step ladder was 34" below the scaffold plank access. In addition, the scaffold frame and crossbrace were used to gain access. Violation observed on or about 05/11/12.
