Electrical protective equipment was not subjected to periodic electrical tests in accordance with Tables I-5 and I-6: (a) Type 1 Class 0 White Rubber Co. rubber insulating gloves were not tested every six months after first issue. The gloves are used by the in-house electrician when performing voltage checks after powering down equipment and before performing work. On or about 5/29/2012 the gloves were used when troubleshooting and replacing the PC of machine #2 (230 volts). Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about 5/30/2012, (a) The horn on the Caterpillar 50 CN-786 forklift was not functioning, exposing the operator and employees in the southwest section of the building to struck-by hazards associated with a lack of ability to provide an audible warning and therefore safe operation of a forklift. The forklift was not removed from service despite the need for repair. (b) The horn, brake lights, and head lights on the Yale 671383 forklift were not functioning, exposing the operator and employees in the northeast section of the building to struck-by hazards associated with a lack of ability to provide audible and visible warnings and therefore safe operation of a forklift. The forklift was not removed from service despite the need for repair. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
