Water, Wichita, KS, multiple pieces of equipment were missing point of operation guards, exposing employees to a caught in hazard in the following instances:    1) HYMAC Roll Bag Converting Machines #2, 3, and 4, located on the production floor.    2) HYMAC T-Shirt Machines #1, 2, 4 and 5, located on the production floor.    3) Birmingham Mill located in the maintenance area.    4) Birmingham Lathe located in the maintenance area.

Water, Wichita, KS, in the maintenance shop, the Dayton Bench Grinder had the work rest on the right side adjusted to approximately 1/2 inch opening, exposing employees to a struck by hazard from grinded debris and debris was an exploding grinding wheel.

Water, Wichita, KS, in the maintenance shop, the Dayton Bench grinder had the tongue guard on the left wheel adjusted to approximately one inch, exposing employees to a struck by hazard from grinded debris and debris from a wheel explosion.
