Employees were observed working from the fourth work platform level, on or about 05/23/12

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a) Jobsite - 1702 Latona Street- A ladder or other safe means of access was not provided. Employees were using the side frames and cross bracing to access the working levels. Obsreved on or about 05/23/12

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows: a) Jobsite - 1702 Latona Street - The working levels of the tubular welded scaffold were not fully planked between the front and rear uprights. Employees were working on the 2nd & 3rd levels of the scaffold. Observed on about 05/23/12.
