29 CFR 1910.157(e)(3):     Portable fire extinguishers were not subjected to an annual maintenance check:    On or about May 23, 2012, and at times prior there to, the employer did not ensure that the portable fire extinguisher in the northeast corner of the wash room was subjected to an annual maintenance check, exposing employee's to fire and smoke inhalation hazards.

29 CFR 1910.305(c)(4):     Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface.    On or about May 23, 2012, and at times prior there to, the employer did not ensure that the snap switches mounted in boxes in the storage room, had faceplates installed so as to completely cover the opening and seat against the finished surface, exposing employee's to an electrical shock hazard.
