29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:    a) Jobsite: 933 Bainbridge St, Philadelphia, PA. - On or about 5/25/12, the working platforms on the supported tubular welded frame scaffold were missing planks. Employees installing facial boards on the 3 story residential building were working from the 4th and 5th platforms, approximately 33 ft. above ground level.

29 CFR 1926.451(b)(5)(i):     Each end of a platform 10 feet or less in length extended over its support more than 12 inches.    a) Jobsite: 933 Bainbridge St, Philadelphia, PA. - On or about 5/25/12, the cantilevered portion of the platforms on the supported tubular welded frame scaffold extended more than 12 inches over their support which exposed the employees to tip over hazards. Employees installing facial boards on the 3 story residential building were working from the 4th and 5th platforms, approximately 33 ft. above ground level.
