29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) The written energy control lockout procedures were inadequate in that they did not have a provision for annual lockout inspections, did not address locking out during cleaning of the carcass saw, and did not have a provision for dealing with outside contractors. (b) The written energy control (lockout) procedures were inadequate in that they implied that tags alone were acceptable, but no additional precautions or procedures were develop to ensure that tags accomplished the same degree of protection and exclusive control that locks provide. (c) The energy control procedures were inadequate in that the locks were keyed the same and did not provide exclusive control for securing machines that were locked out.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: (a) There was inadequate energy control (lockout) training as employees were not trained to lockout the main power disconnect for the Kentmaster carcass splitting saw when cleaning it as they just turned the main power disconnect in the adjacent room to the off position.
