29 CFR 1926.304(d):  Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe.      On or about May 25, 2012, on the south side of a construction site, employees engaged in framing activities on a residential building utilized a Makita skill saw without the lower guard exposing employees to amputation hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems.      On or about May 25, 2012, on the north side of a construction site, employees were engaged in framing activities on a residential building without fall protection.  The employees were exposed to a fall hazard of up to 18 feet to the ground below.
