29 CFR 1910.106(d)(3)(ii)(a):     Metal storage cabinets construction. The bottom, top, door, and sides of cabinet were not at least No. 18 gage sheet iron and double walled with 1 1/2 - inch air space. Joints were not riveted, welded or made tight by some equally effective means. The door was not provided with a three-point lock, and the door sill was not raised at least 2 inches above the bottom of the cabinet.    On or about 16 May 2012, employees were exposed to fire and chemical hazards resulting from flammable materials being stored in a cabinet near the northeast corner of the maintenance bay area that was not constructed of at least 18 gage sheet iron, was not double walled with a 1 1/2 inch air space, did not have a door provided with a three-point lock, and did not have a door sill that was raised at least 2 inches above the bottom of the cabinet.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    On or about 16 May 2012, employees at the Mid-Cities Motor Freight facility were exposed to chemical hazards resulting from the lack of an effective, written hazard communication program that describes how container labeling, material safety data sheets, training, information, and other required critera will be conveyed to employees.
