29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    (a).  Worksite located at 18200 Hidden Valley Road, Independence, Missouri- Employee was working in a trench and near equipment overhead and was not protected from impact by the use of any means of head protection.

29 CFR 1926.651(d):     Exposure to vehicular traffic. Employees exposed to public vehicular traffic shall be provided with, and shall wear, warning vests or other suitable garments marked with or made of reflectorized or high-visibility material.    (a).  Worksite locateda at 18200 Hidden Valley Road, Independence, Missouri- Employees were working right next to a road way in whch the barricades had been knocked down by vehicles and the men were not wearing reflective vests.

29 CFR 1926.651(k)(1):     Daily inspections of excavations, the adjacent areas, and protective systems shall be made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection shall be conducted by the competent person prior to the start of work and as needed throughout the shift. Inspections shall also be made after every rainstorm or other hazard increasing occurrence. These inspections are only required when employee exposure can be reasonably anticipated.    (a).  Worksite located at 18200 Hidden Valley Road, Independence, Missouri- Employees were exposed to trench collapse hazards in which a competent perrson had not inspected the  trench to ensure the trench was safe for employees to work.

Worksite located at 18200 Hidden Valley Road, Independence, Missouri- An employee performing a water main tap was exposed to a trench collapse hazard and was not protected from collapse with an adequate shielding system.
