29 CFR 1926.451(b)(1):     Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports:    On June 1, 2012, an employee was allowed to work from a scaffold that was not fully planked, exposing the employee to a fall hazard of 19.5- 20.5 feet.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(c)(1)(ii):     Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide.    On June 1, 2012, an employee was working from a scaffold that was not tied in to the building, exposing the employee to the hazards associated with a scaffold collapse.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1):     Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On June 1, 2012, an employee was not protected from a fall while working from a metal frame scaffold, exposing the employee to a fall hazard of 19.5- 20.5 feet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    On or about June 1, 2012  employees were exposed to hazardous chemicals such as Portland cement and silica.  The company did not develop a hazard communications program which includes training, labeling and material data sheets.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
