29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b) At the job site, on June 1, 2012 employees were exposed to falls of over 30 feet while laying composite shingles on a 4/12 pitch roof of an apartment building with no fall protection being used.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder. At the job site, on June 1, 2012, employees were using a ladder which did not extend 3 feet above the landing surface and was not properly secured at the top to allow access to a low sloped roof (4/12) where employees were installing composite shingles.
