29 CFR 1910.106(e)(2)(iv)(a): Flammable liquids were not kept in covered containers when not actually in use: Waste Gasoline Storage Bus - Bung hole screw caps were not installed on ten 55 gallon metal drums, and the top was removed from one 55 gallon metal drum. There were approximately ninteen drums on the waste gasoline storage bus.

29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors: Waste Gasoline Storage Bus - Adeqate precautions were not taken to prevent the ignition of flammable vapors. On June 4, 2012 at approximately 4:45 pm one of two employees in the waste gasoline storage bus was smoking a cigarette, causing flammable vapors to ignite.
