The employee was approximately 12 feet above the ground, and did not reconize the hazard of the fall and means to abate. The employee was not trained, on or about 6/5/12. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) Worksite, front of garage - Employee used an unsecured portable ladder to access the garage roof. The ladder was approximately 2 inches above the upper landing, on or about 6/5/12.
